Hubungan Kadar Vitamin D, Sel T Regulator (CD4+Cd25+Foxp3+) dan Kadar C-Peptida pada Anak dengan Diabetes Melitus Tipe 1

Abstract

Diabetes melitus tipe 1 bersifat multifaktorial dan bergantung pada kombinasi kompleks dari elemen genetik, epigenetik, molekular dan selular yang menyebabkan kerusakan dari toleransi perifer sehingga menghasilkan kerusakan pada sel β pankreas. Beberapa penelitian menunjukkan kadar vitamin D yang rendah pada penderita DM tipe 1. Penelitian ini dilakukan secara cross sectional dangan subjek penelitian penderita DM tipe 1 usia 5-18 tahun, dan kontrol subjek sehat dengan usia yang sama. Kadar vitamin D plasma dan C-Peptida diukur dengan metode ELISA sedangkan jumlah sel T-regulator diukur dengan metode flowcytometry. Perbandingan antara kadar vitamin D, jumlah sel T regulator, dan kadar C-peptida  dianalisis dengan uji t tidak berpasangan. Hubungan antara vitamin D plasma, jumlah sel T regulator dan C-peptida dianalisis dengan uji korelasi Pearson. Penelitian ini menggunakan 15 sampel dan 15 kontrol. Kadar 25(OH)D3 secara signifikan lebih rendah pada penderita DM tipe 1 (22,07±5,53 vs 32,88±1,81ng/ml; p<0,05). Jumlah sel T regulator pada DM tipe 1 lebih rendah secara bermakna dibandingkan kontrol (6,86±1,20 vs 37,86±9,61; p<0,05), demikian juga kadar C-peptida (0,3±0,11 vs 3,40±1,10; p<0,05). Terdapat korelasi positif yang kuat dan signifikan antara vitamin D3 dengan jumlah sel T regulator dan kadar C-peptida, dan antara jumlah sel T regulator dengan kadar C-peptida pada penderita DM tipe 1. Dapat disimpulkan bahwa penderita DM tipe 1 mempunyai kadar vitamin D yang lebih rendah dibandingkan normal, dan kondisi ini berhubungan dengan jumlah sel Treg dan kadar C-peptida.Kata Kunci: Diabetes mellitus tipe 1, C-peptida, sel T regulator, vitamin D3
